2009 Registration

To participate in the STARtalk Turkish Summer Camp, you must complete the following registration forms.

You must complete all forms and submit them to Norman Leonard at nleonard@projects.sdsu.edu before your child is officially enrolled in the Turkish Summer Camp. The deadline for these forms is July 15, 2009.

• Parents need to complete the Final STARtalk Application Form for Parents
• Students need to complete the Final STARtalk Application Form for Students
• Students under age 18 need to complete the Student Cover Letter.
• Students under age 18 need to complete the Student Consent Form.
